Monthly Child Placement Census Form for Licensed Providers
- The following forms are to be utilized by Residential Child Care Programs and Child Placement Agency Programs-Treatment Foster Care (TFC) and Independent Living Programs (ILP) licensed and/or contracted by the Department of Human Resources.
These forms are to completed by the 10th of each month (reporting on the prior month)
for each of your programs and submitted electronically to
ssa_census@dhr.state.md.us.
